Several years ago when we were remodeling our house into a care home my old time electrician buddy wis-wired a half hot such that the switch was across the line.

When we threw the switch, there was a small pop and of course the circuit went dead. After a bit of troubleshooting we discovered that we lost one line to the building.

SCE was very good at coming right out and replacing the underground feeders from the street. It appears that there had been a tiny nick in the insulation when they were installed, and 45 years of corrosion with 120 volts to ground finally ate through enough of the cross section of aluminum that a fault was enough to blow out the rest.
